The project, called PATH – Proactive & Accessible Transformation of Healthcare, is led by the Guy’s and St Thomas’ NHS Foundation Trust, the largest hospital unit in the UK’s National Health Service, serving over 1.3 million patients.
PATH is designed to reshape how healthcare is delivered across the NHS by integrating cutting-edge AI, remote monitoring, and digital care tools. The initiative brings together a powerhouse of innovators, including Sword Health, Nvidia, General Catalyst, and Hippocratic AI, to tackle one of the system’s most pressing challenges: accessibility and efficiency.
At the heart of PATH is a mission to move from reactive to proactive care, from hospitals to communities, and from analogue processes to fully digital patient journeys. Sword Health’s AI-based care platform, which has already treated over 500,000 patients worldwide, will be deployed to support patients in managing pain, recovery, and overall well-being, while also reducing costs and easing the burden on healthcare professionals.
The first major focus will be addressing the widespread waiting list crisis affecting healthcare systems globally. With growing demand and limited resources, patients often face significant delays for initial consultations and necessary procedures, underscoring the urgent need for more efficient, accessible, and technology-enhanced care. Rather than merely managing backlogs, PATH aims to rethink the entire patient journey: enabling clinicians to prioritize based on clinical need, empowering individuals to take an active role in their recovery, and ensuring smarter, more effective use of healthcare resources.
Through AI-driven support systems, virtual care models, and real-time monitoring, PATH will allow NHS teams to scale their impact without compromising on quality or patient experience. Generative AI agents, designed with safety and clinical reliability at their core, have already conducted over 2.4 million interactions with high satisfaction rates, providing a new layer of responsiveness to patient needs.
